Form Templates’ Insert options let you add headers, footers, page numbers, images, graphics, pages, page breaks, tables, special symbols, hyperlinks, and text frames.

In the New Form/Edit Form window, click Insert:
Add a page at the end of the form.
Start a new page in the form where the cursor is inserted in the form.
In the Insert Table drop-down, either:
You can edit the table on the form using Table Tools.
Insert an image file (.jpg, .png, .bmp, .tif formats) where the cursor is inserted in the form.
Select a shape type, then click on the form to add a default shape, or click and drag across the form to create a custom-sized shape.
To modify the illustration:
Insert Hyperlink – To add a link to a web page (start with http:// or https://) or email address (start with mailto:).
Edit Header – To add content to page headers
Edit Footer – To add content to page footers
Insert Page Number – To add page numbers to the open header or footer.
You can edit the contents in the header/footer using the tools in Home, Insert, and Data Fields.
Insert Text Frame – To create a text box in the form, select this, then click and drag area for text.
Show Text Frame Marker Lines – If you have no-line text boxes, select this to see the text box areas on the form.
You can edit the appearance of text frames using Frame Tools.
[Special symbols] – Click a symbol to insert it into the form where the cursor is.
Format the appearance of symbols using the tools in Home.
